New Status Reporting Form

  • The new Status Report template provides a single consolidated form for reporting of the following project events:

    o Project Startup

    o Status Report

    o Change Request (previously known as Project Amendment)

    o Conditions Met notification

    o Project On Hold notification

    o Project Resumption notification

    o Project Completion notification

    o Project Cancellation notification

Beginning July 1st, GITA will be requiring Agencies to use the new PIJ documents and Oversight/Monitoring forms for all new projects.

These forms are now available for your use.  Additional feedback is always welcome. 

Arizona Revised Statute, 41-3504 Powers and Duties of the Agency, requires the Government Information Technology Agency (GITA) to evaluate and monitor Information Technology (IT) projects, including expenditures, activity reports and to perform periodic reviews. This document contains information and reference to documents used by the Project Monitoring Group to review the progress of IT projects and investments for agencies of the Executive Branch.

After projects are approved, GITA is responsible for periodic progress reviews that track the major milestones and deliverables. GITA reviews outstanding issues and may make recommendations for improvements or corrections to the project. Focal points of the review include: project scope, management, value to the public, roles, responsibilities, change control, cost containment and utilization of resources.

The Agency Business Unit provides GITA with periodic status reports on all major or critical projects that are reviewed by GITA. The frequency of the reporting is based upon the complexity of the project. These reviews help ensure the projects are being managed in an appropriate manner, and sound business practices are being followed from IT and financial perspectives.

Through effective leadership, GITA endeavors to plan, coordinate, direct and monitor IT projects toward successful implementation using life cycle analysis and to ensure the State’s projects are completed on time and within budget. This is the main thrust and goal of GITA Project Oversight Group.
